Top Tips - Application Form
If you complete all parts of your application correctly, the Tribunal will not need to contact you to find out more information. This means that the Tribunal staff can process your claim more efficiently and reduce your waiting time.
You should especially take care to provide:
- An accurate but brief description of the act of violence. (N.B. You should not say 'refer to the police statement');
- The correct date or dates of the act of violence;
- If the matter was reported to Police you should tell us the date it was reported, the name of the Police Officer who took your complaint and where the Officer was stationed; and
- Details of the compensable injury you are claiming to have received. A compensable injury is an injury listed in the Schedule of Injuries. If you have not named a compensable injury, then the Tribunal will send you a Bridging Form which you will have to complete and return.
