Civil forms - General

If there is no relevant approved form, approved Form 1 is to be used and adapted as necessary. It may be useful to refer to the content of the old Supreme, District or Local Court forms when preparing a document for which there is no specific approved form.

* You must include information about the main type of claim you are making in the Statement of Claim, Summons or Notice of Appeal.
