Before you download the application for admission (Form 10) below, please note the following essential requirements for admission applications.

We have been experiencing a large number of applications that are being submitted incomplete. You should note that any admission application that is incorrect or incomplete at the application deadline will not be accepted for the admission ceremony. These applications will be held over to the next ceremony or until the application is complete and correct.

Please ensure that you fully complete the declaration requirements as set out in each form.

If you agree with the declaration place your signature in the adjacent box. If you do not agree with the declaration place an 'X' in the adjacent box and attach the relevant disclosure.
