    - Where the transcription is less than 3 months old, a deposit of $73.00 plus $8.90 per page.
    - Where the transcription is more than 3 months old, a deposit of $89.00 plus $10.20 per page.

      • Subpoena for Production Use UCPR Form BUT - Clause 75(1), Dust Diseases Tribunal Regulation 2007 states "the requesting party...must endorse a proposed access order on the subpoena" (ie, UCPR Form #28 - Subpoena Notice - is not required). The proposed access order should be endorsed on the front sheet, if possible.
      • Advice to Practitioners No. 4 of 2005 (Forms for New Asbestos Claims)
        N.B. The forms in the Advice to Practitioners No. 4 of 2005, must also follow the additional requirements of the Uniform Civil Procedure legislation.
