Assessment Report Format (December 2011)
This comprehensive report format allows everything to be included in the one document. This version reflects our transfer to Sydney Children's Hospitals Network and includes the correct clinic address and other contact details. It automatically numbers paragraphs and pages to allow easy reference.

Risk of Significant Harm Reports to Community Services (Department of Human Services)
If you discover that a child is at risk of harm, as a mandatory reporter you must make a report to Community Services. The best way is by telephone on 133 627. We also suggest that you access the resources for mandatory reporters. This includes an interactive, online Mandatory Reporter Guide to be used to help decide whether a report is warranted. With the proclamation of the Children Legislation Amendment (Wood Inquiry Recommendations) Act 2009 No 13 on 24 January 2010 this is necessary to help you organise the important information to allow an effective response from Community Services. When you call 133 627 you will be asked what you found from the online guide.
