How to apply
Before you begin filling out the compensation claim form, you should check to see if you are eligible for compensation.
You may complete and send your application for compensation online. Please remember to save your form before you press the submit button.
The one application form can be used if you are applying for Compensation and/or Expenses under the Victims Assistance Scheme and/or Counselling.
Use of this form requires that Adobe Reader (8.01 or above) be installed on your computer. The free Adobe Reader software can be downloaded from the Adobe website.
At the end of the form you are asked to press a SUBMIT button which will send the application to Victim Services. If the submission process has been successful you will see on your screen a copy of the form with an enquiry number near the top. See sample of a submitted form

NOTE: It currently takes 25 months between lodgment and determination of a claim by a compensation assessor.
